PERIODONTAL (GUM DISEASE) UPGRADE

  • Patients requiring periodontal maintenance can upgrade their membership plan.
  • Perio Upgrade: +$250/year
  • Includes 3–4 periodontal maintenance visits per year (as recommended by your doctor) in place of standard cleanings.
  • This upgrade is recommended for patients with a history of gum disease to maintain optimal oral health and prevent progression.
  • Scaling and Root Planing (deep cleaning) is not included but is eligible for the standard membership discount.
  • For patients requiring periodontal treatment, preventive benefits may be applied toward necessary care as determined by the doctor.

  • This is a discount membership plan and not dental insurance.
  • Valid only at Arrow Dental and cannot be used at outside specialists.
  • Discounts apply to dental services only and not retail products.
  • Membership becomes active on the date of enrollment and renews annually.
  • This plan cannot be combined with insurance or other promotions.
  • Unused benefits do not roll over to the next year.
  • Discounts may be adjusted if using third-party financing such as CareCredit.
  • Membership fees are non-refundable regardless of usage.
  • Plans are non-transferable between individuals or families.
  • Payment is due at the time services are rendered.
